It is the policy of the Company that all requests for changes/updates to shareholder accounts must be in writing to:

Change in Registration

• A valid stock certificate(s) of the shares to be transferred.

• Medallion Signature Guarantee

• Completed Stock Power

Death of a Shareholder (Joint Tenants)

• Certified Copy of the Death Certificate

• Valid Stock Certificate

• W-9 Form

• Affidavit of Domicile

Lost Certificate- All lost certificates must be reported in writing.  They may be reported using the Notification form (below).
