Premium Payment Options

We offer several convenient options to pay your premiums. 

• You choose the frequency of premium payments. Premium payments can be made annually, semi-annually, quarterly or monthly. Premium billings are mailed approximately 20 days prior to the premium due date.
  
  To change your premium-billing mode, contact our Customer Service Department at 800-323-0050 or click here to download and print a Multipurpose Change Form. Please complete section 7, provide the appropriate signatures and return the original form to our office.

• You choose the method. Premium payments will be accepted by check or they can be paid automatically from your bank account. Or, take advantage of the convenience of on-line premium payments. Click here if you would like to make a payment by debit from your checking account or credit card.

If you would like to have your premiums paid automatically from your bank account, click here to download and print a Request for Electronic Funds Transfer. Fill out the form completely and mail the original to our office. Please include a voided check or deposit slip from the account you will be using.
