All forms require the signature of the policy owner and an unrelated adult witness. Because original signatures are required to process any changes to your policy, forms must be returned to us by mail.

Multi-purpose Change Form: Use this form to make changes to your policy, such as name, address, beneficiary, ownership, premium billing frequency or dividend option.

Request for Electronic Funds Transfer: Complete this form if you would like to have your premium payments paid automatically from your bank account. Please be sure to include a deposit slip or voided check when you return the form to our office.

Loan Form: Complete this form if you would like to request a loan against the current cash value of your policy. Be sure to answer the Notice of Replacement question and provide your social security number.

Proof of Death Claimant Statement: Use this form to file a claim on active policies. Please contact us to verify beneficiary information or call our Customer Service Department at 800-323-0050.

Indemnification Agreement (sample form) 
Indemnification Agreement: Use this form if an estate has been designated as a beneficiary and the estate has been closed.
